The problem is with the missing XmlElement and XmlAttribute attributes. A public property will serialize as an element by default unless you tell it to serialize differently. I removed those attributes from a test service I have and the xsd I returned from that service was the same as yours, all elements.
Those attributes are in the SDK in sushi.cs, so somewhere along the way I'd guess that your file was regenerated or altered along the way.
